Community Based Care, LLC (“CBC”) acquired Lindley Habilitation Services on April 19th, 2017. Community Innovations will operate independently with its current management team as a wholly owned subsidiary of Community Based Care, LLC. Founded as a subsidiary to Saguaro Management, Inc., Community Innovations will continue under the leadership of its current CEO, Hal Jones, an original member of the founding Saguaro group.
Community Based Care, LLC is a holding company whose aim is to expand its footprint across the Intellectual and Developmental Disabilities (“I/DD”) market in North Carolina, and will be looking to aggressively grow across the state both organically, and acquisitively. Gene Rodgers, an industry veteran of over 20 years, will be augmenting the current management teams of each entity, and leading the overall strategy, and corporate business development efforts for the Holding Company.

Adding CI’s unique service lines and geographic coverage fills out the continuum of care among our holdings, which will help us better serve people with I/DD and the insurers with whom we contract.”
CBC will look to capitalize on the favorable industry tailwinds behind this space by allowing smaller providers in the space the opportunity to continue performing excellent clinical work while providing them with an opportunity to join a large organization with a scaled infrastructure to handle administrative issues.

Community Innovations, Inc. (CI) was established in 1988 to address the needs of North Carolina’s intellectually and developmentally disabled (IDD) population under the referral and triage care of the many Area Programs for Mental Health/Developmental Disabilities/Substance Abuse governmental agencies in the state. CI provides adult residential and supported living services, community based periodic services, day programming, and other services.
Community Innovations has 5 offices in North Carolina providing services and various programs for adults and children with special needs. Its services span half the state and reach out to individuals from Trillium Health Resources, Alliance Behavioral Healthcare, Eastpointe, and Sandhills Center MCO catchment areas.

Community Based Care, LLC is a holding company formed to acquire individual agencies that provide home and community based care for the I/DD population. CBC currently has two holdings.
CBC is interested in exploring additional opportunities throughout North Carolina and surrounding states. CBC has initially acquired HomeCare Management and now with the purchase of Hughes Behavioral Health completed is working on additional opportunities. The agencies provide services to the I/DD populations, predominantly through the I/DD Innovations Waiver.
