Community Based Care, LLC (“CBC”) acquired Lindley Habilitation Services on 12/3/16. Lindley Habilitation Services will operate independently with its current management team as a wholly owned subsidiary of Community Based Care, LLC. Neil Lindley, the founder and CEO of the agency is a 25+ year veteran of the industry and a very-well respected provider of community based services who will pursue other interests upon completion of the sale.
Community Based Care, LLC is a holding company whose aim is to expand its footprint across the Intellectual and Developmental Disabilities (“I/DD”) market in North Carolina, and will be looking to aggressively grow across the state both organically, and acquisitively. Gene Rodgers, an industry veteran of over 20 years, will be augmenting the current management teams of each entity, and leading the overall strategy, and corporate business development efforts for the Holding Company.

CBC will look to capitalize on the favorable industry tailwinds behind this space by allowing smaller providers in the space the opportunity to continue performing excellent clinical work while providing them with an opportunity to join a large organization with a scaled infrastructure to handle administrative issues.

Since 1998 Lindley Habilitation Services specializes in helping persons with Intellectual and Developmental Disabilities reach their fullest potential. We provide Innovation/IDD Medicaid in home services, Day Program Services, AFL Services, as well as private pay care for the Innovations I/DD community.
Lindley Habilitation Services has 6 offices in North Carolina and 1 office in Florida providing services and various programs for adults and children with special needs. Its services reach out to individuals from the Trillium Health Resources, Alliance Behavioral Healthcare, Sandhills Center, and Cardinal Innovations Healthcare Solutions MCO catchment areas.

Community Based Care, LLC is a holding company formed to acquire individual agencies that provide home and community based care for the I/DD population. CBC currently has two holdings.
CBC is interested in exploring additional opportunities throughout North Carolina and surrounding states. CBC has initially acquired HomeCare Management and now with the purchase of Hughes Behavioral Health completed is working on additional opportunities. The agencies provide services to the I/DD populations, predominantly through the I/DD Innovations Waiver.
